The Application Process »

To receive an application for riding, call 985-796-3677, email ntherapy@bellsouth.net or complete the form on the Contact Us page of this website.

All parts of the application, including the doctors authorization, must be completed and mailed to New Heights, P. O. Box 1283, Folsom, LA 70437.  Upon receipt of the application, New Heights staff will contact you to schedule an evaluation.  All applicants are evaluated by staff.  A Physical or Occupational Therapist participates in the evaluation for all who have a phyiscal disability.
