To register for two sponsor levels, follow the steps below for each level separately.

1. 1) Complete the form below and submit it
2. 2) Review your information
3. 3) Make any necessary changes, using the back button of your browser and re-submit the form.
4. 4) to pay by Master Card, VISA or through PayPal, click the "Buy Now" button below your information to complete the transaction.  If you need to make any changes in your information, click the back button of your browser before you choose to complete the transaction on the PayPal screen. You will receive a Thank you and receipt from New Heights, but for your protection, print the receipt from the PayPal payment screen for your records. You will not be re-routed to New Height website after your transaction is complete.
